The information requested helps Your Perfect Footwear Foundation provide much needed resources to our Participants and ensure future funding. This information is confidential. Information recieved via this form regarding individuals is not made public in any way.

APPLICATIONS ACCEPTANCE IS NOW CLOSED FOR SEPTEMBER.

next month's event.

Availability is limited to 50 students monthly.
Applicants who have submitted requirements but were NOT selected will be added to the waitlist for future consideration as funds are available.
Applications per child will only be accepted every 12 months.
